Low (Low to Moderate Stretch)
PTT uses a medical traction device to apply gentle, sustained stretch to the penis over a controlled period. This continuous mechanical tension is designed to stimulate a natural cellular process called mechanotransduction — where tissue responds to sustained stretch by remodelling and generating new cells.
It’s the same principle used in orthopaedics and reconstructive medicine for decades: tissue placed under consistent, controlled tension adapts and remodels over time.
PTT is not a quick fix, a gimmick or an over-the-counter gadget. Clinical-grade traction protocols have been examined in published studies for Peyronie’s disease, penile curvature and length preservation after prostate surgery — and they require correct fitting, correct tension and a structured protocol to be done safely.
PTT may form part of a treatment plan for men experiencing:
Penile curvature and Peyronie’s disease — sustained traction is designed to counteract the shortening and curvature caused by scar tissue (plaque) in the penis. Published studies have examined traction protocols for reducing curvature in men with Peyronie’s disease.
Loss of length after prostate surgery — penile shortening is a common but rarely discussed consequence of prostatectomy. Traction protocols have been studied as part of post-surgical penile rehabilitation, designed to help preserve tissue length during recovery.
Tissue shortening and retraction — whether from scar tissue, disuse or reduced blood flow, shortened erectile tissue can affect both function and confidence. PTT is designed to support tissue remodelling over time.
As a complement to shockwave therapy — traction and shockwave target different aspects of tissue health. Combining them is designed to address both the structural (scar tissue, shortening) and vascular (blood flow) components of penile health.
Consider booking an assessment if you:
Curvature and shortening tend to progress if left unaddressed — and the earlier tissue changes are assessed, the more options you have. Many men wait years before speaking to someone. There’s no need to be one of them.
We don’t hand you a device and send you home. PTT at Shockwave Clinic is delivered as part of a structured, clinically guided programme — often in combination with our in-clinic technologies:
Traction addresses the structural component. Shockwave and EMTT address the tissue and vascular components. Used together, they’re designed to work on the problem from multiple angles — rather than temporarily masking symptoms.
What treatment involves:
Individual responses to treatment vary. Your assessment will determine whether PTT is suitable for you, and which combination of therapies fits your situation.
We treat before we recommend. Your first visit isn’t just a consultation — it includes your first full treatment session, so we can see exactly how your body responds before building your plan.
Step 1 — Initial Assessment & Treatment (R5,450 once off) includes:
During your assessment, we learn:
This information helps us create a treatment roadmap tailored specifically to you — including whether PTT should form part of your plan, and at what stage. No awkwardness. No pressure. Just clear answers about what’s going on and what your options are — all handled confidentially.

More Than Pills, A Better Way Forward
If you compare erectile dysfunction to something like tennis elbow, the logic becomes clearer.
With tennis elbow, you would not rely only on ibuprofen or a cortisone injection. These may reduce pain temporarily, but they do not repair damaged tissue or restore normal function. Real recovery comes from treating the underlying cause, improving blood flow, and stimulating healing.
We take the same approach with erectile dysfunction.
Tablets and surgery often manage symptoms, but they rarely address the root problem, which in most cases is reduced blood flow and weakened penile tissue. That is why we focus on regenerative treatments such as shockwave therapy, together with supportive therapies designed to restore natural function by improving circulation and encouraging tissue repair.
Our goal is not short-term symptom control.
Our goal is long-term recovery and healthier erectile function, using treatments that work with your body rather than override it.
